$100 (one-time payment, per patient)
Monthly/Quarterly/Annually Payment Options for Memberships!
Monthly Membership:
$99/month (per patient)
Quarterly Membership:
$279/quarterly (per patient)
Annual Membership:
$999/year (per patient)
The registration fee is a non-refundable enrollment fee to sign up as a patient in the practice.
