This DPC membership includes open access to Dr. Chris for any medical service, so you can obtain weight loss and/or hormone replacement services, along with all the benefits of your DPC membership for primary care!

$100 (one-time payment, per patient)
Monthly/Quarterly/Annually Payment Options for Memberships!
Monthly Membership:
$99/month (per patient)
Quarterly Membership:
$279/quarterly (per patient)
Annual Membership:
$999/year (per patient)
The registration fee is a non-refundable enrollment fee to sign up as a patient in the practice.
